目的:
探讨

F-FDG PET/CT在恶性胸膜间皮瘤(malignant pleural mesothelioma,MPM)与胸膜转移性腺癌鉴别诊断中的关键表现及其在临床分期中的价值。
方法:
回顾性对比分析7年来因原因不明胸痛、胸膜增厚、胸腔积液行
FFDGPET/CT检查,后经病理证实的MPM和胸膜转移性腺癌患者各20例,比较两组患者胸腔积液程度、胸膜FDG积聚形态、SUV
max
及全身伴随征象,并判定MPM的TNM分期。
结果:
MPM组多为无或少量胸腔积液。胸膜FDG积聚形态类型比较发现,MPM组纵隔胸膜FDG积聚者明显多于胸膜转移性腺癌组(70%
vs
20%,
P
0.01),而两组间结节型、肿块型、不规则环形,以及叶间胸膜、横膈胸膜FDG积聚的发生率差异无统计学意义,两组间胸膜SUV
差异也无统计学意义。全身伴随征象中,发生肺门和纵隔淋巴结转移者MPM组明显少于胸膜转移性腺癌组(25%
80%,
0.05),而心包、胸壁和肋骨侵犯,同侧胸廓体积缩小在MPM组中明显增多(50%
0.05;75%
25%,
0.01)。MPM组临床TNM分期中,Ⅰ~Ⅱ期为60%,Ⅲ~Ⅳ期为40%。
结论:
F-FDG PET/CT发现纵隔胸膜FDG积聚是MPM与胸膜转移性腺癌鉴别诊断的关键表现,同时在MPM临床TNM分期中也发挥着重要作用。
Objective:
To analyze the value of
F-FDG PET/CT in differential diagnosis of malignant pleural mesothelioma (MPM) and metastatic pleural adenocarcinoma and clinical staging of MPM.
Methods:
The
F-FDG PET/CT appearances of 20 cases with MPM and 20 cases with metastatic
pleural adenocarcinoma confirmed pathologically over 7 years were retrospectively reviewed
at the same time
the clinical TNM staging of MPM was investigated.
Results:
Pleural effusions were none or a little in MPM. Of pleural thickening uptake types
mediastinal pleural involvement was significantly different (
0.05). While nodular pleural thickening
mass pleural thickening
rind-like pleural irregular thickening
fissure pleural involvement
diaphragm involvement and maximum SUV
were not significantly different (
0.05). Mediastinal and (or) hilum lymph node enlargement
invasion of pericardium
chest wall and rib
decrease in the volume of affected thorax between the two groups were significantly different (
0.05). Clinically
Ⅰ-Ⅱ stage occupied 60%
Ⅲ-Ⅳ stage occupied 40% in MPM.
Conclusion:
Mediastinal pleural involvement is a critical appearance in differential diagnosis of MPM and metastatic pleural adenocarcinoma.
F-FDG PET/CT has important value in clinical TNM staging of MPM.
