摘要

目的：

探讨乳腺浸润性导管癌（breast invasive ductal carcinoma，IDC）超声声像图特征与雌激素受体（estrogen receptor，ER）、孕激素受体（progesterone receptor，PR）及增殖指数Ki-67阳性率之间的关系，探索超声技术能否为IDC治疗方案选择及预后评估等提供依据。

方法：

回顾性分析2014年6月2016年12月收集的79例IDC患者的超声声像图特征（肿块大小、形态、边缘、钙化、淋巴结转移及血流信号等），以及ER、PR、Ki-67的阳性率情况，分析IDC超声声像图特征与ER、PR及Ki-67阳性率之间的关系。

结果：

IDC免疫组织化学检测结果显示，ER阳性47例（59.5%，47/79）、ER阴性32例（40.5%，32/79）；PR阳性46例（58.2%，46/79）、PR阴性33例（41.8%，33/79）；Ki-67阳性55例（69.6%，55/79）、Ki-67阴性24例（30.4%，24/79）。IDC超声表现为边缘毛刺征及未发生淋巴结转移与ER、PR阳性率增高有关（

＜0.05）；肿块内微小钙化、血流显像分级呈富血供及发生淋巴结转移与Ki-67阳性率增高有关（

＜0.05）；而瘤体大小、形态与ER、PR、Ki-67不相关（

＞0.05）。

结论：

IDC超声声像图特征与ER、PR、Ki-67阳性率相关，超声技术可便捷地为IDC诊断、治疗方案选择及预后评估等提供更多的参考依据。

Abstract

Objective:

To investigate the correlation between ultrasonographic features and the positive rates of estrogen receptor (ER)

progesterone receptor (PR) and Ki-67 in breast invasive ductal carcinoma (IDC).

Methods:

The ultrasonographic features of 79 cases with breast IDC confirmed by patholog

y were analyzed retrospectively

including tumor size

edge

shape

calcification

blood flow signal

lymph node metastasis

etc. Immunohistochemical method was used to detect the positive rates of ER

PR and Ki-67. The correlations between ultrasonographic features and ER

PR and Ki-67 positive rates were analyzed.

Results:

The positive rates of ER

PR and Ki-67 in breast IDC were 59.5%

58.2%

69.6%

respectively. Spicular sign on ultrasonographic images and negative axillary lymph node metastasis were correlated with high positive rates of ER and PR (

＜0.05). The microcalcifications

blood flow grade 2-3 and positive axillary lymph node metastasis were correlated with high Ki-67 proliferative index (

＜0.05). Tumor size and shape were not correlated with the expressions of ER

PR and Ki-67.

Conclusion:

There are certain correlations between ultrasonographic features and the positive rates of ER

PR and Ki-67. Sonographic findings could indicate biological behaviors of IDC in some ways

and provide more evidence for the degree of malignancy and prognosis of IDC.
