摘要

目的：

探讨乳腺实性乳头状癌（solid papillary carcinoma，SPC）的超声表现，并与乳腺导管内乳头状瘤（intraductal papilloma of the breast，BIDP）进行鉴别诊断。

方法：

回顾并分析2015年1月2020年3月在南京医科大学第一附属医院经手术后病理学检查证实的65例乳腺SPC及77例BIDP的临床表现、超声特点，并对两者进行鉴别诊断。

结果：

乳腺SPC与BIDP在年龄、病变类型、病变距乳头距离、肿块大小、回声均匀度、腋窝淋巴结有无转移、乳腺影像报告和数据系统（Breast Imaging Report and Data System，BI-RADS）分类方面差异有统计学意义（

＜0.05）。与BIDP相比，乳腺SPC患者发病年龄较大，多表现为单发实性肿块，患者的触诊率高，肿块距离乳头距离较远，肿块偏大，多内部回声不均匀，易出现腋窝淋巴结转移，BI-RADS分类升至4类概率提高。而在内部回声、边缘、形态、方位、有无钙化、后方回声、Adler分级、阻力指数（resistance index，RI）、是否伴有导管扩张方面差异无统计学意义（

＞0.05）。

结论：

乳腺SPC的超声表现具有一定的特征性，结合患者的发病年龄、病史及临床表现，有助于通过超声检查鉴别诊断乳腺SPC与BIDP。

Abstract

Objective:

To investigate the ultrasonographic features of solid papillary carcinoma (SPC) of the breast and differential diagnosis from intraductal papilloma of the breast (BIDP).

Methods:

The ultrasonographic features and clinical manifestations of 65 patients with SPC and 77 patients with BIDP confirmed by pathology in the First Affiliated Hospital of NJMU from January 20

15 to March 2020 were retrospectively reviewed. Differential diagnosis was performed.

Results:

There were significant differences in age

lesion type

distance from the nipple

size

uniformity of the echo

axillary lymph node metastasis

Breast Imaging Report and Data System (BI-RADS) grades between SPC and BIDP（

＜0.05）. Compared with BIDP

the majority of SPC patients showed the older age

single solid mass

higher palpation rate

longer distance from the nipple

SPC has certain ultrasonographic features. Combing with the age
