摘要

目的：

探讨国产uMI 780型正电子发射体层成像（positron emission tomography，PET）/计算机体层成像（computed tomography，CT）仪在临床应用

F-FDG进行全身（脑部+体部）显像时的最佳图像采集及处理条件。

方法：

收集2017年8月—2018年5月符合要求的受试者400例，将400例患者随机纳入预先制订的由

F-FDG注射量2.96 MBq/kg和3.70 MBq/kg、脑部PET显像时间3 min和2 min、体部2 min/床位和1.5 min/床位三者组合形成的4组（A1、A2、B1、B2，每组100例）研究序列。按照优（3分）、良（2分）、差（1分）对脑部和体部PET图像的代谢分布清晰度、设备性伪影、图像均匀性进行视觉评价，比较

F-FDG注射量、显像时间和重建算法或重建参数组合对脑部和体部PET图像质量的影响。

结果：

脑部PET图像质量有序子集最大期望值法（ordered subset expectation maximization，OSEM）均明显优于滤波反投影法（filtered back projection，FBP）（

＜0.001），而不同

F-FDG注射量及显像时间组间差异无统计学意义。当

F-FDG注射量为2.96 MBq/kg时，体部PET显像时间2 min/床位的代谢分布、图像均匀性均明显优于1.5 min/床位（

＜0.05），而设备性伪影两者差异无统计学意义；当注射量为3.70 MBq/kg时，显像时间2 min/床位和1.5 min/床位间图像质量差异无统计学意义；当显像时间为2 min/床位时，注射量3.70 MBq/kg和2.96 MBq/kg间的图像质量差异无统计学意义；当显像时间为1.5 min/床位时，注射量3.70 MBq/kg在代谢分布和图像均匀性上呈现比注射量2.96 MBq/kg更好的趋势；应用OSEM算法重建体部PET图像时，迭代2次的代谢分布和图像均匀性明显优于迭代3次（

＜0.01），而对设备性伪影无显著影响；高斯半高宽（full width at half maximum，FWHM）为4 mm的代谢分布和图像均匀性明显优于3 mm的图像（

＜0.01），而图像设备性伪影仅个别组别呈现4 mm明显优于3 m

m的图像（

＜0.01）。

结论：

研究得出国产uMI 780型PET/CT仪临床应用

F-FDG的最适图像采集和处理方案共两个：① 注射量2.96 MBq/kg；脑部显像2 min，OSEM重建；体部显像2 min/床位，OSEM重建的迭代次数为2、高斯FWHM为4 mm。② 注射量3.70 MBq/kg；脑部显像2 min，OSEM重建；体部显像1.5 min/ 床位，OSEM重建的迭代次数为2、高斯FWHM为4 mm。

Abstract

Objective:

To investigate the optimal acquisition and reconstruction protocol of uMI780 positron emission tomography (PET)/computed tomography (CT) in the clinical application of

F-FDG for whole-body (head+body) imaging.

Methods:

A total of400 subjects from August 2017 to May 2018 were collected. The 400 subjects were applied with two levels of FDG injection dose

3 or 2 min for brain scan

2 or 1.5 min/bed for body scan

OSEM and FBP resulted brain images

and 2- or 3-iteration OSEM body images with two Gaussian kernel for post-smoothing. Image quality was evaluated qualitatively and quantitatively and scored by two clinical physicians.

Results:

The OSEM image quality was significantly better than that FBP (

＜0.001)

but there was no significant difference in brain PET image quality between different injection doses or different scan times. When the injection doses of

F-FDG was injected with 2.96 MBq/kg

image quality of body PET imaging with 2 min/bed were significantly better than the 1.5 min/bed (

＜0.05); but

when the injection was 3.70 MBq/kg

there was no difference in image quality of body between 2 and 1.5 min/bed. When scan time was 2 min/bed

there was no significant difference in between image qualities of using 3.70 MBq/kg and 2.96 MBq/kg; but for 1.5 min/bed

the image quality of 3.70 MBq/kg was better than 2.96 MBq/kg. Two-iteration OSEM image quality of body were significantly better than that of 3 iterations (

＜0.01). Image quality by using 4 mm FWHM Gaussian was significantly better than 3 mm Gaussian (

＜0.01).

Conclusion:

By the study

could obtain two optimal image and reconstruction protocols of

F-FDG for uMI780 PET/CT in clinical application: ① 2.96 MBq/kg injection dose; 2 min brain scan

2 min/bed body scan with 2-iteration OSEM reconstruction with 4 mm Gaussian smoothing; ② 3.70 MBq/kg injection dose; 2 min brain scan

1.5 min/bed body scan with 2-iteration OSEM reconstruction with 4 mm Gaussian smoothing.
