摘要

目的：

比较常规超声和超声弹性成像对肛瘘、肛周脓肿的诊断价值。

方法：

选取上海市杨浦区控江医院2014年4月2016年4月收治的80例肛瘘、肛周脓肿患者，术前分别进行常规超声和超声弹性成像检查，并与手术后病理学检查结果比较，评价这两种超声方式诊断内口、主管、支管及脓腔等的效果。

结果：

术前常规超声与超声弹性成像均能清楚显示瘘管的主管道，两者诊断准确率均为100%；超声弹性成像对内口定位的诊断准确率为95.0%（76例），明显高于常规超声诊断的85.0%（68例），差异有统计学意义（

0.05）；超声弹性成像对支管及脓腔定位的诊断准确率为96.3%（77例），明显高于常规超声诊断的86.3%（69例），差异有统计学意义（

0.05）；超声弹性成像对肛瘘分型的诊断准确率为93.8%（75例），明显高于常规超声诊断的83.8%（67例），差异有统计学意义（

0.05）；超声弹性成像的灵敏度、特异度、阳性预测值及阴性预测值等明显优于常规超声，差异有统计学意义（

0.05）。

结论：

常规超声与超声弹性成像均为术前检查肛瘘、肛周脓肿的有效方法，超声弹性成像具有更高的准确率。

Abstract

Objective:

To study the value of conventional ultrasound and ultrasonic elastography in the diagnosis of anal fistula and perianal abscess.

Methods:

From Apr. 2014 to Apr. 2016

80 cases with anal fistula and perianal abscess in Kongjiang Hospital of Yangpu District

were selected. All patients were examined by conventional ultrasound and ultrasonic elastography before operation

Results:

Both conventional ultrasound and ultrasonic elastography could clearly display the main fistula

with the accuracy of 100%. Ultrasonic elastography had higher accuracy (76 cases

Conclusion:

Both conventional ultrasonography and ultrasonic elastography are effective methods for preoperative examination of anal fistula and perianal abscess

and ultrasonic elastography has higher accuracy.
