目的:
快速有效地评估肺癌治疗效果。
方法:
采用肺部CT灌注图像的半自动分割方法和计算机辅助诊断方法对肺癌血管靶向治疗效果进行评估。首先读入肺部DICOM格式的CT灌注图像,然后采用改进的距离规则化水平集分割方法(modified distance regularized level set,MDRLS)对肺部肿瘤进行分割,将分割得到的区域逐层映射到其他CT灌注参数图像上,通过选取合适的阈值,分割出肿瘤的血管丰富区域,再分别计算肿瘤和内部血管丰富区域的体积。采用5例患者治疗前后的CT灌注参数图像,通过计算豪斯多夫距离,分析图像分割的效果;通过比较治疗前后肿瘤和血管丰富区域的体积,评估肺癌血管靶向治疗效果。
结果:
采用MDRLS可以得到较好的图像分割结果,疗效评估结果也与临床病理数据相吻合。
结论:
该研究提出的MDRLS方法有望对肺癌血管靶向治疗效果进行有效地评估,从而为肺癌的临床治疗提供重要参考。
Objective:
To quickly and effectively evaluate the therapeutic efficiency of lung cancer.
Methods:
Semi-automatic segmentation method was proposed for lung cancer in CT perfusion images and the computer-aided diagnosis (CAD) method was applied to evaluate the efficacy of vascular-targeted therapy. First
CT perfusion images of lung cancer in DICOM format were input and the modified distance regularized level set (MDRLS) algorithm was used to segment lung tumors. Then the segmented results were mapped to other CT perfusion images. By selecting the appropriate threshold
vessel-rich areas of the tumor were obtained.
The tumor volume and its vessel-rich areas were calculated respectively. Experiments were conducted with CT perfusion images in five patients before and after therapy. With the calculation of Hausdorff distance
the performance of the segmentation method was analyzed. With the comparison of the tumor volume and its vessel-rich areas before and after therapy
the therapeutic efficacy of vascular-targeted therapy was evaluated for the lung cancer.
Results:
The results showed that the proposed method achieved good segmentation performance and the therapeutic evaluation results also agreed with the pathological data.
Conclusion:
Therefore
the proposed method MDRLS may evaluate the efficiency of vascular-targeted therapy for lung cancer
which will provide the significant reference for the clinical treatment.
