目的:
探讨儿童骨肉瘤经过新辅助动脉化疗后,肿瘤微波原位灭活联合放射性
125
I粒子植入术在保肢、保骺治疗中的临床疗效。
方法:
自2011年3月2016年9月,采用微波原位灭活联合放射性
125
I粒子植入术治疗19例儿童四肢关节周围骨肉瘤。其中男性11例,女性8例,平均年龄8岁。Enneking肿瘤分期,ⅠB期3例,ⅡA期2例,ⅡB期14例,平均随访47个月。
结果:
16例采取了保留骨骺的微波灭活联合放射性粒子植入术。随访期间,18例存活,1例肺转移死亡,所有患儿均未发现肿瘤局部复发,术后无感染、皮肤坏死,术后未发生灭活瘤段骨骨折。根据Enneking骨骼肌肉肿瘤术后肢体功能评定标准,平均功能评分为86.7%(26分/30分),优良率达89.5%。
结论:
微波原位灭活治疗儿童四肢关节周围骨肉瘤可获得优良的肢体关节功能,可使骨骺未被侵犯的儿童骨肉瘤患者保留更多骨骺、骨量及软组织覆盖。同时,放射性125I粒子植入可有效降低肿瘤的局部复发率,是一种创新、安全的保肢术。
Objective:
To investigate the clinical eff icacy of microwave inactivation combined with
125
I seeds brachytherapy in osteosarcoma in children following effective neoadjuvant intra-arterial chemotherapy.
Methods:
From Mar. 2011 to Sep. 2016
19 children who were suffering from limb osteosarcoma
they were eleven male and eight female patients
average 8 years old
ⅠB for 3 cases
ⅡA for 2 cases
ⅡB for 14 cases according to Ennekings surgical staging system. All patient
s received neoadjuvant intraarterial chemotherapy before surgery
and received microwave inactivation in situ combined with 125I seeds brachytherapy. Allograft bone or bone cement f illing was adopted for repairing the bone defect
and the bone affected was f ixed by using internal f ixation. The mean follow-up was 47 months.
Results:
One patient died of lung metastasis. There was no local infection
recurrences
fracture
looseness or rupture of internal fixation
and no radiation injury of blood vessels or nerves. According to evaluation criteria for postoperative function of skeletal muscle tumors in Enneking
the average functional score was 86.7% (26/30) and the excellent and good rate was 89.5%.
Conclusion:
On the premise of effective neoadjuvant chemotherapy
the microwave inactivation combined in situ with
125
I seeds brachytherapy can result in satisfactory limb length and joint function for children with osteosarcomar. Meanwhile
as a type of brachytherapy
125
I seeds implantation could effectively prevent local recurrence after surgery.
