目的:
探讨

F-FDG PET/CT的代谢参数对初诊转移鼻咽癌患者预后的预测价值。
方法:
对20102018年初诊即诊为远处转移,在治疗前行全身
F-FDG PET/CT检查的85例鼻咽癌患者进行随访,以最大标准摄取值(maximum standard uptake value,SUV
max
)的40%及SUV的绝对值为2.5分别作为阈值,用软件计算肿瘤代谢体积(metabolic tumor volume,MTV;MTV为病灶SUV高于阈值的体积)和体积内SUV
mean
,计算总病变糖酵解(total lesion glycolysis,TLG;为MTV和SUV
乘积)。分别计算鼻咽、颈部肿瘤、全部转移灶及全身所有病灶MTV和TLG,分别定义为NPMTV2.5、NPMTV40,NKMTV2.5、NKMTV40,MMTV2.5、MMTV40,WBMTV2.5、WBMTV40及NPTLG2.5、NPTLG 40,NKTLG2.5、NKTLG 40,MTLG2.5、MTLG40,WBTLG2.5、WBTLG40。用R软件的survivalROC包计算上述指标,预测2年总体生存率(overall survival,OS)的最佳截断值,Kaplan-Meier法行单因素分析评估各指标的预后预测价值,Cox模式多因素预后分析。
结果:
单因素分析发现转移部位、化疗周期、放疗剂量、MSUV
、NKMTV40、MMTV2.5、WBMTV2.5、MTLG40、WBTLG40是影响OS的因素,多因素分析提示化疗小于4个周期、MMTV2.5>13.04 mL(HR=2.46,
P
=0.02),MTLG40>43.74 g(HR=2.54,
=0.01)是影响OS的独立不良预后因素。
结论:
初诊远处转移鼻咽癌转移灶的
F-FDG PET/CT的MMTV2.5和MTLG40是独立的影像学预后因素,具有较高的预后价值。
Objective:
To invest
igate the prognostic value of metabolic parameters of
F-FDG PET/CT in patients with de novo metastatic nasopharyngeal carcinoma.
Methods:
85 patients with de novo metastatic nasopharyngeal carcinoma diagnosed from 2010 to 2018
who had
F-FDG PET/CT before treatment were enrolled. The threshold values of standard uptake value (SUV) were 40% SUV
and absolute value 2.5 respectively. Metabolic tumor volume (MTV) above the threshold and SUV
in the volume werecalculated by software. Total lesion glycolysis (TLG)=MTVSUV mean . MTV and TLG of nasopharynx
neck tumor
all metastases and all lesions in the whole body were calculated respectively
which were defined as NPMTV2.5

the predictive value of each index was evaluated by Kaplan Meier method
and the multivariate prognostic analysis of Cox model was conducted.
Results:
Univariate analyses showed that metastasis site

WBMTV2.5 were strongly related to OS. The multivariate analyses identified that less than 4 cycles chemotherapy
MMTV2.5 >13.04 mL (HR=2.46
=0.02)
MTLG40 >43.74 g (HR=2.54
=0.01) were independent negative prognostic parameters for OS.
Conclusion:
MMTV2.5 and MTLG40 of
F-FDG PET/CT of de novo metastatic nasopharyngeal carcinoma patients are independent prognostic factors of imaging and have high prognostic value.
