To investigate the application value of energy spectrum CT in preoperative staging of T
3
and T
4
gastric cancer.
Methods:
The clinical data of 60 patients with advanced gastric cancer confirmed by operation and pathology hospitalized in Longhua Hospital
Shanghai University of Traditional Chinese Medicine from October
2015 to October 2017 were analyzed retrospectively. All patients underwent energy spectrum CT scan within one weak and the results were compared with the pathological results.
Results:
The pathological results confirmed 19 cases with T
3
stage and 41 cases with T
4
stage. The sensitivity
specificity and accuracy of energy spectrum CT based on serosal changes in diagnosis of T
3
and T
4
carcinoma were 85.5%
81.2%
84.6%
respectively. The sensitivity
specificity and accuracy of energy spectrum CT based on perigastric fat plane in diagnosis of T
3
and T
4
carcinoma were 81.5%
66.6%
76.4%
respectively. There was no difference in the overall accuracy between the two CT features. The iodine concentration in the perigastric fat tissue adjacent to tumor in T
4
carcinoma was significantly higher than that in T
3
carcinoma
and the difference was more obvious in venous phase than that in arterial phase.
Conclusion:
Energy spectrum CT is of high application value in the preoperative staging of gastric cancer. It can provide a scientific basis for the reasonable preoperative chemotherapy for the patients with gastric cancer.
